Register

To register, please pick up a patient registration pack from reception. These forms are also available online, below. An online service is coming soon. If you have any queries, any member of our staff will be happy to help you either at the surgeries or on the telephone.

Adult Registration – Please can you complete the following and return to the receptionist.

  2. New patient application form
  3. GMS1 Registration Form (purple form)
  4. Important information about your detailed Health records – Please Read
  5. Your Electronic Patient Record & the Sharing of Information – Please Complete
  6. Changes to your Records – Please read

Child Registration – Please can you complete the following and return to the receptionist.

  1. New Child Patient Information Letter
  2. GMS1 Registration Form
  3. Summary Care Record – your emergency care summary – Pease Read
  4. Important information about your detailed Health records – Please Read
  5. Your Electronic Patient Record & the Sharing of Information – Please Complete

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

As you know from the TV news, Coronavirus can spread very quickly through human contact.
GP surgeries, where there are Doctors and Nurses caring for sick people is a place where you could be at risk of being infected. We need to concentrate all our efforts on dealing with patients who need our help the most. Only come to the surgery, if one of our practice health team call you to arrange an appointment.
